Liberty Bankers Life Insurance Company (“Liberty Bankers”) and The Capitol Life Insurance Company (“Capitol Life”) have assumed the Covered Obligations arising out of the insolvency and liquidation of Lincoln Memorial Life Insurance Company effective November 1, 2017.

Also, effective November 1, 2017, the Special Deputy Receiver (“SDR”) for National Prearranged Services (“NPS”) assigned the Run-off Responsibilities on all NPS prepaid funeral benefit contracts and/or other prepaid funeral benefit contracts acquired by NPS that are associated with these Lincoln Memorial Life policies.

Landmark Admin LLC is acting as a third-party administrator for Liberty Bankers and Capitol Life related to the Lincoln Memorial Life policies.

If you determine that you need to file a claim for covered obligations assumed by Liberty Bankers or Capitol Life, you must use the appropriate claim forms below.

  • Please note that not all Covered Obligations for Lincoln Memorial Life have been assumed. Use the link below to determine if your policy number or contract number has been assumed by Liberty Bankers or Capitol Life and is being administered by Landmark Admin LLC.
  • If you determine that your policy meets these guidelines, return to this page and print the forms you need to file a claim (state forms + W9). We cannot accept claims forms as used by the SDR’s office.  Claims submitted must be on our current forms.
